Here are the forms and stationery items that supplement the IMP in your school.

End of Year 6 Certificate - download this document and save it, then type in student names on the certificates and keep printing.

Year 5 Pack - A collection of start up paperwork for Yr 5 band classes and teachers.

Consumables Order Form - For all reeds, oils, etc

A price list and order form for use by the IMP's client schools. Check date at bottom of form to make sure it is current. Can be completed and emailed, faxed or just handed to IMP staff. 

 

Year 5 Selection - A multi page document explaining the procedure to be adhered to by client schools in the selection of Year 5 students for the band class. Fife and Recorder schools see separate page for fife/recorder for your version.

 

Instrument Loan Agreement - This is a sample of an agreement that may be put into place between a student's family and the school to define the terms of membership of a band class and financial responsibility for the borrowed instrument. A good idea for all. Here is a second version that may suit your school.

 

Year 5 Percussionists - A special welcome to Year 5 percussion players explaining to them and their parents just how important they are to the band and how hard they will have to work to learn both tuned and untuned percussion.

 

Practice Record - Even though we include a practice record in each student journal there may be an occasion where a special record sheet is required. Here it is.

 

Work Experience - On the rare occasions that the IMP accepts work experience students here is the job application form for work experience applicants.
